Liability Waiver / Release of Authority

I, the undersigned parent/guardian, grant permission for **[STUDENT NAME]** to attend the field trip to **[DESTINATION]** on **[DATE]**.

I understand that **[SUPERVISOR]** will be supervising the trip. In the event of an emergency, I authorize the attending supervisor to seek medical treatment for my child, including transportation to a medical facility. I hold the school harmless from liability arising out of this trip.
